The Nighttime Blood Pressure Dip

In healthy adults, blood pressure drops by 10 to 20 percent during sleep. Researchers call this nocturnal dipping. It happens because the body shifts from the sympathetic nervous system, which drives the fight-or-flight stress response, to the parasympathetic system, which governs rest and repair. The heart beats more slowly. Blood vessels relax. The kidneys process more fluid. Blood pressure falls.

A landmark study by Verdecchia and colleagues, published in Hypertension, found that non-dippers had a significantly higher rate of cardiovascular events than dippers, even when their daytime readings looked the same. Nighttime blood pressure is, in many ways, a better predictor of long-term heart risk than the reading taken at a clinic.

Why Some People Do Not Dip

Several conditions reduce or eliminate the overnight dip. Sleep apnea is the most common. Each apnea event triggers a brief stress response that spikes blood pressure and heart rate. People with untreated sleep apnea may have dozens or hundreds of these events per night. Chronic kidney disease, diabetes, autonomic nervous system dysfunction, and high sodium intake all impair dipping. So does poor sleep quality, even without a diagnosed sleep disorder. Age also plays a role. The dipping pattern tends to flatten with age, partly because deep sleep decreases and partly because the autonomic nervous system becomes less responsive.

What Disrupted Sleep Does to Daytime Blood Pressure

Missing or shallow overnight blood pressure dips do not just affect nighttime readings. They carry over into the next day. The sympathetic nervous system, denied its overnight rest, stays more active during waking hours. Stress hormones, particularly cortisol and adrenaline, remain elevated. Blood vessels do not relax as fully. The net result is daytime blood pressure that runs consistently higher than it would with good sleep.

Research also supports timing medication with sleep in mind. A large trial by Hermida and colleagues, published in the European Heart Journal, found that people who took at least one blood pressure medication at bedtime had significantly better cardiovascular outcomes than those who took all their medications in the morning. One proposed mechanism is that bedtime dosing better supports the overnight dip. This is an area worth discussing with your physician.
