What the Oslo randomized controlled trial found

A randomized controlled trial published in Blood Pressure, led by researchers at Oslo University Hospital, enrolled 118 adults with high-normal blood pressure or stage 1 hypertension. Systolic readings ranged from 130 to 159 mmHg. Half the group ate three kiwifruits per day for eight weeks. The control group ate one apple per day. The researchers chose apples because they also contain antioxidants and fiber, making it a meaningful comparison rather than a no-treatment placebo. At the end of eight weeks, the kiwifruit group showed lower 24-hour ambulatory systolic pressure by 3.6 mmHg and diastolic by 2.5 mmHg compared with the apple group. The reduction held across both office and ambulatory measurements.

That 3.6 mmHg systolic reduction is clinically meaningful. A 3 mmHg reduction in systolic blood pressure corresponds to roughly a 10 percent reduction in stroke risk across population studies. The kiwifruit effect, sustained over eight weeks with no other dietary changes, sits in the same range as some antihypertensive lifestyle interventions. Why kiwifruit lowers blood pressure: three mechanisms

The researchers measured several biomarkers alongside blood pressure. Plasma lutein levels rose significantly in the kiwifruit group. Antioxidant capacity also increased. Nitric oxide metabolites did not change significantly, suggesting the blood pressure effect did not primarily run through nitric oxide bioavailability. The proposed mechanisms involve three overlapping contributions.

First, lutein and carotenoids reduce oxidative stress in blood vessel walls. Lutein is an oxycarotenoid with high antioxidant capacity. Oxidative stress damages the endothelium and drives the arterial inflammation that elevates blood pressure over time. Kiwifruit is one of the richest commonly consumed sources of lutein alongside green leafy vegetables.

Second, vitamin C supports endothelial function. Vitamin C contributes to collagen synthesis in blood vessel walls and acts as a direct antioxidant in plasma. Adequate vitamin C levels support the structural integrity of arteries and help maintain healthy endothelial function. A single green kiwifruit delivers 55 to 65 mg of vitamin C. Third, potassium contributes to the sodium-potassium balance. A single medium green kiwifruit delivers approximately 215 mg of potassium.
